Forms Required for Registration

All schools of choice students, post-secondary students and those seeking a Mature Student High School Diploma must print and fill out the Application Form and Permission to Release Information Form. Please do not mail forms to ATC. To register, you must meet with the ATC counsellor. Please call 237 8951 to make an appointment.

Please note, the ATC office is closed between July 6th and August 17th 2007.

LRSD high school students and students attending Sunrise and DSFM schools must see the student services department at their home school for the appropriate registration form.

Authorization to Publish Photos

All ATC students must print and complete the Authorization to Publish Photos Form to indicate whether they do or do not authorize the Louis Riel School Division to publish photographs, videos or audio recordings of themselves (son or daughter if under 18) for educational purposes and for the promotion of learning.

Transportation of Students on Approved Off-School Site Programs

Students who will be transporting other students in their vehicle on approved off-school site trips must print and complete the Volunteer Driver Form prior to the event. Only one form per school year is required.
